Photography:  $20 for each quilt.   If you are using a photographer other then Frank Nored, check here____. 

Entry Fee:  $15 for each entry.  Attach check for the entry fee and photography made payable to AQuA.  I have attached my check in the amount of $______which is for photography and entry fee.  Or, I have attached my check in the amount of $______as I am using a different photographer.  Note, only one check is required for multiple pieces.  If you are submitting multiple pieces, check here____.  If my work is juried out I still want it photographed.______

Insurance:  Entry of the above items means that I fully understand that AQuA members will take every precaution to protect my quilt in the gathering, shipping, and redistribution, but I understand that they cannot be responsible for acts of nature beyond their control.  As an entrant, my signature grants AQuA and the Denver National Quilt Festival

and its designated sponsors the right to photograph my quilt for publication, advertising or promotional use.
